What is the Lifetime Assurance Plan?
A LASIK guarantee in three steps
While perfection cannot be guaranteed, the Lifetime Assurance Plan helps you maintain your personal best vision throughout your life.
Have your procedure
Complete your laser vision correction at a participating LASIK Vision Institute Eye Center, along with the full course of post-operative care your doctor determines is medically appropriate.
Enroll in the plan
After completion of your laser vision correction and upon approval by your independent LASIK Vision Institute surgeon, you can elect to enroll in the Lifetime Assurance Plan.
Covered for life
If you experience visual changes related to your original refractive error, you receive the same-technology re-treatment free of charge, when deemed medically appropriate by the surgeon.
Eligibility requirements
Who qualifies for the plan.
Lifetime Assurance Plans are available to all who are qualified by a LASIK Vision Institute surgeon to enroll at the time of their laser vision correction procedure.
- Qualified at the time of your procedure. The plan is available to those who are qualified by a LASIK Vision Institute surgeon to enroll at the time of their laser vision correction procedure.
- Complete post-operative care. Patients must have followed the full course of post-op care their doctor determined to be medically appropriate.
- Medically appropriate re-treatments only. Participating surgeons will only perform enhancement procedures that are medically appropriate for your eyes.
- Re-evaluation before any enhancement. Prior to using your plan, the surgeon re-evaluates your vision and your eyes to ensure you’re a good candidate for an additional LASIK surgery.
- Related to your original correction. The covered re-treatment must be related to a change in the refractive error originally treated: myopic, hyperopic, or astigmatism.

What the plan doesn’t cover
The re-treatment covered in the Lifetime Assurance Plan must be related to a change in the refractive error originally treated: myopic, hyperopic, or astigmatism.
Certain changes in your visual system may affect your ability to have an enhancement procedure.
Eye disease
Changes in vision caused by eye diseases such as cataracts, glaucoma, macular degeneration, or diabetes are not covered under the plan.
Accident or unrelated conditions
Reductions in vision due to disease, accident, or other unrelated permanent or temporary conditions are not eligible for re-treatment.
Other eye surgeries
Correction of refractive error resulting from cataract surgery, or other eye surgeries like retinal surgery, is not covered under the Lifetime Assurance Plan.
The LASIK Vision Institute Lifetime Assurance Plan is available only at participating LASIK Vision Institute Eye Centers to qualified patients. Please inquire at the Eye Center nearest you for additional details.
